ASEBP is pleased to announce the introduction of an updated Extended Health Care (EHC) and Vision Care claims form, effective January 15. This update is part of ASEBP's ongoing commitment to providing efficient and secure services to covered members.
Why the New Form?
Your experience is ASEBP's top priority. The new claims form enhances security by providing ample space for attaching receipts, ensuring all necessary documents are securely submitted. In addition, the form will feature a new mailing address specifically designed to speed up the claims process. This streamlines processing and demonstrates ASEBP's dedication to making things easier and more convenient for you. With itemized sections for receipts and claims, the form ensures all necessary information is clearly provided, minimizing errors, reducing processing delays, and improving the overall efficiency of handling claims.

To assist with online submissions, ASEBP has created a helpful "General Claims" interactive video that guides covered members through the process, step by step.
Grace Period for Old Forms
Understanding that transitions take time, ASEBP is offering a grace period until July 15, 2025, during which ASEBP will continue accepting the old claims forms. This grace period will allow covered members time to try out the online claim’s submissions and the new paper form before ASEBP retires the old form. (Note: Always make sure you sign the paper form to prevent delay in processing times)
